What happens if a Gardevoir or any other Ralts-line Pokémon loses one of its spikes or otherwise has it become damaged?
They couldn't sense people's emotions via telepathy any more, they'd have to be near a person to understand what's actually going on. Which could be a problem if the person or Pokémon they're near is angry.

It's a part of them so I'm sure it'd hurt quite a bit, maybe like breaking a bone but also becoming emotionally retarded in the process. They might be able to function but since it's cited the source of their abilities they probably wouldn't do very well if at all in the wild.
