The Red and Green artwork is as old as the games.

Koffing only has the skull above his face in Japanese Blue, which is what Western Red and Blue use as source.

I think the intention with the earlier sprites was that not every Koffing or every Vaporeon was exactly the same- like, you know, some Koffing have their jolly roger on their chest, others might have it on their head, and so on, but the GB didn't have that kind of memory.

At least that's how I picture it
