IIRC it's because the "Ultimate" stage was thought by the tamers to be the strongest stage they could reach. "Mega" was to do the impossible and reach a stage further beyond, something that required a vague prophecy to be fulfilled.

Every game before featured levels up to Ultimate. Before the anime Phoenixmon, MetalSeadramon and HerculesKabuterimon were known and listed as Ultimate. It wasn't dub, they literally used the anime to introduce new level to the franchise when the right episode aired.

Depends. There was a male Biyomon in Data Squad. Basically when it comes to genders it's hard to set any rules except that there are two of them. Daily reminder that Renamon line was male in German dub. And there was a monument depicting a male Bastetmon in Xros Wars. And a Nanimon referred to as female in one of DS games.

Basically the rule of thumb is that it can be either/or unless clearly feminine/masculine in design and/or name and if it's design only there may exist an alternate gender form within the same species.

That's because the PS1 game came first, and there weren't any mega forms back then. You had the two baby forms, Rookie, Champion and Ultimate. The anime was supposed to be a 12 episode short series that became a hit surprisingly. Problem is that the names were established already, so they came up with two new evolutions: Mega and DNA Fusion.

The game was supposed to be popular and the anime not, but ended up being the opposite. The next game for DW2 added the Megas and Fusions, but they fucked up by making an entirely new game instead of polishing the last one, which fucking sucks because the Digimon games could have been what Pokémon is today.
