If I remember right, it was the toy pokeball that killed him. He fell asleep with half of it over his mouth/nose or something and couldn't breathe.
So they removed the pokeball part of the toys but kept giving out the part people actually wanted (the Pokemon).
My brother had a Golbat and a Nidoqueen, I had the Mew and Ditto.
