Happened to me twice (maybe even three times, I don't remember) in one battle with a fucking Goldeen. I was too dumbfounded to be angry, I wasn't even sure that was possible.

apparently i'm a fucking noob, i though critical captures were when you throw a pokeball and it catches the pokemon without having to go through the animation of the pokeball shaking, how do you tell apart a failed critical capture from a normal failure. other than the normal one shakes a few time before breaking?

That is a critical capture. It shakes midair to let you know it's unlikely to break out, nudges once, and then clicks. A failed critical capture is when it nudges once and then breaks out.
