How should multiple regions be handled?

With avoidance.

By never doing it at all

no more than one extra region because I can't trust GF with level balancing

Introduce two new regions for two versions of one game.
The region you don't start in is the post-game, abandoned by humans and full of high-level pokémon.

By a company other than Gamefreak, they'd just make the not main regions devoid of anything.

Honestly I think they just went overboard with trying to keep the level curve from gettibg out of hand in GS. You could just make it like 1-50 or 60 for the first region and then up to 100 for the second.
