I am on 11.18 PKhex update and it still occurs for me. All i have to do is walk one step and it will teleport me outside wherever i saved.

backup that save you want to fix, do a new game, copy trainer coordinates from the new game to the save you want to fix using PkHex, then reimport the fixed save.
