If you go to Melemele Meadow, you can go to the bottom left corner of the flower field and crawl into a little cave.

For me it seems to just be ten QR code scans to be able to use the Island scan. Scanning a QR code uses up one icon per scan. There's also a cooldown for each scan icon.
A rare national dex Pokemon shows up in a specific area on the Island you're currently located on. I suggest you try to get more PokeRide Pokemon since you might get somewhere like Ten Carat Hill before receiving Tauros.

I just arrived on Akala Island after beating Melemele's Kahuna and yes, you have one hour to find the Pokemon. The game lets you know when the one our time limit is up.

Yeah I'm really curious if you can keep scanning the same QR codes, because if you need to scan 10 QR codes to use it once that's only a little over 70 scans if they don't let you repeat scans.

I know Yokai Watch has a feature like this with QR codes and you can't repeat any, but there were tons of sources of codes constantly coming so you were never left wanting. The difference is that you could also still catch most everything in a single game, there weren't tons of older Yokai to catch from previous games. I'm going to be bummed if we're limited to a set number of scans, because this seems like a fun feature.

Unless of course the QR codes are different between trainers. Like one person's Pikachu QR code being different from another's Pikachu code, based on it's stats/OT or something.

No, you just get a dex entry as if you saw it. For Alolan dex pokemon it means you can find a location where you can encounter a wild one (and starters don't have a natural habitat). You can also use an island scan for every 10 codes, generating a completely random, non-legendary pokemon in a random location of the island you are currently on. You have 1 hour to catch it.

You can get different pokemon depending on the island. For example, I got Marill on the second island, soft reset, went back to the first and had Litwick instead.

But resetting doesn't change the pokemon. Two resets was still Litwick.

I went with Poopmaid (who is boxed now because even jobber frog is stronger than her), and my idea was to use Ribombee against it - sucks when both Castform and the bird could easily get rid of it, and then things were getting dire. In the end I got Fletchinder for Z-move, leveled him to 22 when Stenee got 30 and evolved into Tsereena, which made everything super easy with high fire damage and her stomping everything to death.

Still a nice surprise after XY where everything was press A to win.
