Kingdom Hearts is where I usually see it.

The hardest mode is critical, where enemies do double damage and you get a few extra abilities from the start. One of those abilities lets you choose whether or not you gain experience, so you can really go through the entire game at level 1. Same principle here, just different workarounds to keep the level at 1.
