Regular Z-crystals just require you to know a move of that type to activate. When you click on the Z-button, the regular move will "upgrade" into a Z-move for one turn. I don't think it becomes a fifth move or anything, it just replaces the old one temporarily.
Pokémon-specific Z-moves work the same way, but they require the pokémon to know a SPECIFIC move instead.

It still is a '5th move', but like Megas, it's not permanent nor does it take up a wholly unique slot. The slot is occupied by the crystal, used to power the move.
