Red++ has 205 Pokémon, but only 22 Pokémon that aren't cross-gens of Gen 1 Pokémon.

Pokémon Red Emu Edition has all 151 and only 151 but doesn't have a difficulty increase.

Pokémon Blue Kaizo has all 151 and only 151 but has a huge difficulty increase, not just moderate.

Don't know anything that meets your exact requirements though.

Yes. Just among Gym Leaders, Erika has a Bellossom, Koga has a Crobat, Sabrina has an Espeon, and Giovanni's Gym team has a Steelix and Rhyperior. The E4 also all have at least one Gen 2/4 Pokémon, some being cross-gens and some not (Agatha's team only has one Gen 1 Pokémon out of her five).

A 151 hack with a mix of Pokemon from Gen 1 to Gen 5 which also features some mechanics from future gens such as the physical special split. It gets really hard in the late game and the post game gets really ridiculous with the elite four rematches who have enemy exclusive mons such as speed boost regigigas, regice with 200 base special attack, and flash fire genesect
