Fly, as far as the out of battle implications are concerned, would require the 'mon have enough lift built into their wings to not only carry themselves but ALSO a trainer.
Now yes, there are /some/ who /might/ be able to pull this off, but most are doubtful. Birds? Sure. Dragons? Sure. Bugs? Just because they have wings don't mean that they're hearty enough to be mounts. Same argument could be made for other things as well. Look at Drifloon/Drifblim.

It's because Fly isn't denoting the actual ability to Fly, but rather, the convenience of riding the Pokemon in question through the air. Most Bug-types can't learn Fly because you couldn't ride them.

I figure it's because most bugs are too simple-minded to follow such a specific flight plan. If something is not in their current of perception or ingrained into their instinct (like bees gathering nectar) they have trouble understanding that the thing even exists. The Special stats seem to be associated with the mind (see: Alakazam), so it makes sense that such a Special powerhouse like Volcarona would be an exception.
