Excessive scripts. You don't want a script in everything to play sounds. You especially don't want to script every surface to play a sound when a user moves over it. That would be insane. You personally can add scripts to your avatar to make footstep sounds, though.

Repetitive environmental sounds. It's one thing to run through a place in a game, while other sound effects are playing. It's another to stay in the same spot and hear the same environmental sounds over and over and over, which is generally where you are in SL. Every time I go to a friend's house or area where a song is playing, and it doesn't switch to a different song after maybe 10 minutes, it's getting muted.

Lack of good sound effects for sale/little way to preview them. A lot of sound effects are bad, and at the point where someone can mesh, script, and texture, it's expecting too much of them to be good with sound effects too.

If you build your own location, you can absolutely incorporate as many sounds as you want, and you can script solutions to almost all these issues. However, if you're asking why other people won't add sound, it's because silence is far better than bad or annoying sounds.
