The Mega Drive was strong as fuck when it was up against the NES, and Sega dominated the market for a few years. But then the SNES came out and Sega shit the bed by focusing on add-ons and shit while Nintendo just kept throwing out triple-A after triple-A title. They rushed out the Saturn to try and recreate their successes with pitting the Mega Drive against the NES, but the PS1 blew it out of the water. They never really recovered from that.

The reason the Master System was so strong in Europe is that it was release the same year that the NES. In both Japan and America it was released when the NES was already a big success.
Rushing the Saturn wasn't a problem, the problem was the opposition between Sega of America and Sega of Japan, the first one had a good deal with Silicon Graphics for the GPU (who was ultimately used in the Nintendo 64) but Sega of Japan didn't listen and made their poor choice.
If I remember correctly, there was the same kind of thing for the CPU, being finally used by the PSX.

The reason the Saturn failed, at least in the west, is because, in addition to a series of launch day mishaps, Sega of America pretty much refused to import any games for that system that wasn't 3D. Apart from the obvious issue of missing out on a ton of good games because they weren't 3d, the Saturn wasn't designed with 3d games in mind, and as such making 3d games for it is abit of a pain in the ass, which results in less good 3d games for it, which results in less games getting released, the cycle goes on.

And as everyone knows, a console with no games doesn't sell.
