Is the Hunter's Dream the Moon Presence's dream?

Is the Hunter's Dream a place where your mind goes, leaving your physical body behind? If that's the case, when you turn into a squid or assume Gehrman's place, your body is still left behind until it dies off?

If that's not the case (your body and mind goes to the dream altogether), you're trapped in this dream forever, even as a squid?

They don't belong to anyone in particular, the Hunter's Nightmare is 'below' the Nightmare Frontier and is fed by the curse of Kos upon Byrgenwerth and the Church, but the upper echelons of the Nightmare don't have a particular owner or creator. Micolash is more of a host in the party host sense, the japanese translation of his name is "Micolash, groundskeeper of the nightmare"
