Take exhaust if you're worried about getting gibbed in lane, nobody is going to rush a Karthus with exhaust unless you have a sliver of health. Q's range is enormous and keeps you well away from any kill threats, farm like a madman with that. If you aren't worried too much about dying in lane but you're worried about roams grab teleport and countergank, or alternately use your ult if they fuck up at all and punish. RoA > Rylais or Zhonyas will keep you alive against just about everyone.

After you have two items and level 2 ult force a teamfight at dragon or for a tower. You will absolutely smash their team if the assassin hasn't gone out of control and you can land Qs.
