The entry point was the hallway. Goro would never see any other part of Sae's palace because he would pass through the entry point on his way back to the elevator. Sae was down the hall at this point and she doesn't go anywhere near the door until after Fubata's automated messages kick in.

No one checked the body because Sae was with it the entire time and the plan was always to write up a fake suicide autopsy, so there was no point.

This didn't work in the bad end because Sae did not show Goro the phone. Showing him the phone is what sends him in, because Futaba fucked with the app so it could start up without the user input. Joker and the real world guard are much further down the hall when the app is triggered.

There is no such thing as a one room metaverse. The interrogation room was part of Sae's palace, and they entered her palace from that point. There is no way out of the interrogation area without passing through that point, which causes you to leave the palace.

The police assume the body is still in the morgue. They had no reason to check it at the time. Obviously the body was not going to go vanish but they had 0 reason to suspect that the body was gone due to Sae's interference and Shidou keeping the legitimate parties away from the situation.

The only way to fight the reaper before being super high level is during flu season, during which time he will randomly despair at the start of battle, causing him to die 3 turns later without attacking once.
