It's probably worth just sticking with your current save. Your first playthrough of any purse owner game is gonna suck regardless. Just do what looks fun and enjoy the story, then worry about being efficient with your time on NG+ so you can max all your relationships and shit.

You don't lose time if you do dungeons early, in fact it's better to do them early since apparently you can lose out on relationship events if you wait until the last minute. (Not sure if this is true in 5, but it was that way in 4 and 3.)

Levelling up the doctor seems worthwhile, any relationship that also increases your personal stats is always worth doing.

Keep at least one book on you for train rides.

You can switch languages at the load screen. Try it out, see if you like it better.

First is the hardest in that regard since you don't have any reliable regeneration, your SP is finite. You can get soul drops and some SP drinks from the vending machines to help a bit but that's it.

Later on you have ways to extend your visits. Almost indefinitely if you're clever about it.
