Originally: Koha is a gag comic, the joke is that Takeuchi couldn't stop making new Saberfaces each week. So each new strip of Koha Takeuchi would bullshit how he'd create a new one. In one strip he randomly had Nobu and Okita to fuse, and it created Majin Saber

Later Retcon: Koha had its own sanctioned grail war finally by Nasu, which was semi-serious and semi-joke we could say at best. At the end of the story Okita is facing off against Mecha Hitler, and she wishes on the grail for the power to stop him. She becomes imbued with the counter force and finally ends him.

In a comic strip later on Irisviel asked why Majin Saber's backstory seems a bit different now, Majin says "its because I want to be in FGO eventually"

The "Okita Alter" part is entirely a misleading nickname, she's not actually Alter anything, she's not Bad Okita either. At the end of the story Okita and Nobu move in with Akiha for no reason, as a joke.
