Dva is meant to be a fast reacting offtank and disrupter, matrix is meant to be a spot defense skill so she can escort important teammates or quickly react to sudden changes like a flanker attack or ult push. Bad dvas will either stay rooted to one area or try to flank and get chewed up instantly, but the reality is you need to be a midground between both roles and have good situational awareness to anticipate where you'll need to be next to cockblock the enemy and cause miximum trouble

Well, the problem is you really can't know. I'm a Widow main and even I get that "I think he is doing bad and should switch" feeling when somebody else picks her.

The game is so hectic that it's very hard to keep track of the kill feed and even then it doesn't tell you the whole story. Player XY might never show up in the killfeed but he's the one of gets everybody low so that they can get picked off.

So, what are you supposed to think when you are in a game and you're losing? Who's at fault? Most likely the guy on the hero with the high skill ceiling, I guess, maybe... WHO KNOWS?! And this riles people up and the great shit flinging begins.

The least they could do is display the medals over the portrait for everyone to see.

Oh no, even when flanking I usually wait until my team is nearby, but all the enemy team usually needs to stop me is a single roadhog or even slightest amounts of coordination. When I play as a tank my team's never with me, as support my team never protects me, and as anything else I can't output enough damage.
