Nah I just came up with it in a second of thinking. It's partially inspired by Studio Triumph's hospital NTR trilogy.

In the first game the MC is the son and heir of head of the hospital, is successful and popular doctor, is rich, and marries an ojousama. However he gets ensnared in a trap set by a doctor in the hospital which leads to his wife getting NTR'd.

As a result, in the second game he divorces her and develops ED. A childhood friend of his hears about the divorce and applies to work in his hospital. She explains to him how she originally went to nurse school so she could work together with him but by the time she graduated he already got engaged so she decided to work in a different hospital because it would otherwise be too painful to be around him. They fall in love and she tries to him through his ED. however before they managed to fix his ED she gets NTR'd by a fat otaku patient in the hospital.

The shock from getting NTR'd twice puts the MC into a semi vegetable state in the third and last game, where he is a patent in his own hospital and can't do simple things anymore like pee without assistance. His sister hears about this and flies from America to help take care of him. Despite the MC's warnings and crying and nightmares, she gets NTR'd too and well I won't spoil what the MC ends up as

Sort of.

The method used in the pastebin guide is obsolete and doesn't seem to work with the new game versions. I just did some more testing on it and found that CM3D2.ExternalSaveData.Patcher.dll that comes with Hongfire Patch is causing the game to blackscreen. Removing that along with ModPriorityPatcher.dll should allow you to use Reipatcher with Sybaris.

Now that I know which plugins were breaking it I can finally update the guide.

Ideally it'd be great if we could just not use Reipatcher at all and stick with Sybaris, but because Sybaris is developed by the Japanese and they have no need for a translation plugin we're stuck with it until someone comes up with a better way to translate the dialogue and UI.

The Reipatcher translation plugin is nice in that it lets you make your own translations on the fly just by editing text files, but having to repatch is shit.
