I know Bloodborne gets shat on for being too ambiguous sometimes and not giving real answers but it actually gives you plenty of reasonable scenarios to pick from to form lore theories and I think that's the closest thing to what Miyazaki intended these game's stories to be, him having a story with chunks of information unavailable and forming reasonable stories out of it with your imagination.

He's turning into a beast, by the time you meet him, he's practically on the verge of the turning and he does so in the fight.

I think some people experience beasthood differently. For instance, random villagers start turning and gain beastly traits like the usual collapsed pupils, elongated limbs, claws, hair, etc. Then some people like Gascoigne start just getting bigger (he was probably a pretty bigguy4u to begin with). But there's also people like Vicar Amelia who retains her normality until she can't hold it in any longer. I think Gascoigne was like this, but to a lesser extent. As an active hunter, he was slipping a little more every time, adding to his exaggerated size.

It's also a gameplay thing, to make the enemies bigger and more intimidating and easier to focus on.
