Get hit a small number of times and get a rank s or higher. I think Master is less than 15 hits taken and an S rank clear. I know King is 10 and an SS, Slayer is less than 5 hits taken and SSS clear. All of them are unlocked from clearing with the above conditions on the difficulty preceding them.
