After beating the game I think I have a better idea of how it works now and it's really quite simple:

Just click a fuckload to win. You want to keep your arrow high as fuc and when you do that the enemy basically can't do anything unless they're Lady the last bitch who spams the shit out of a whip move that you have to stand and take or the Pink Knight Girl.

Along with what the others said, MV also has issues with screen refresh rates that are not 60hz. I have a 144hz screen which makes it run at double speed for most of the time, except for when it randomly decides to run at 1x speed for a few seconds.
I've read that the developers put out a patch that fixes the issue, but it's a patch that game makers need to apply on their side. If they release a game before applying the patch, the bug will persist.
