The only "always max" skill is Samediveve. Everything else depends on your build and goals.

PvP relies on maxing Effigy, Samediveve, Mackangdal and Hexing. You usually get MVP because you're one of the few cleric classes capable of good sustained DPS from a range that doesn't rely on the enemy standing on a specific spot like a retard. When the enemy has Bloodletting, you play defensively and support your allies until it expires. You're still a cleric, after all.
For PvE, assuming you go the PD/PD2 path you should have no trouble solo'ing your way through R8 with a hexing and effigy combined with plague vapors and incineration.
I have little experience with zombies, it's not very often played in PvE and it's pretty much nonexistant in PvP.

Monsters in PvP, pretty good with PvE once you become a PD.

For PvP it's hard to say. Most R8 classes have onehitKO skills now, but you have Mackangdal.

Supposedly lycanthropy is based on your physical stats, so I guess stats wise you'd want dex since the base attack of the werewolf is very high.

Out of the classes you picked, I think Bokor would be the strongest base for a Lycanthrope build. Mackangdal is 20 seconds of invincibility provided you cancel the end damage with sterea troph, and samediveve is a permanent +7 move speed with divine might.

Priest could be a good base too but Stone skin and blessing, two of their best buffs, scale with Spr very hard, which you won't have since you'll want to keep your max SP low to make SP regen via potions to werewolf take less time. Krivis is more of an Int nuker/support and wouldn't do as well for a lycanthropy focused build.

However you can cast all your skills as a werewolf and people say the hedgestone gives double p.attack/m.attack, so you could still use the werewolf to do typical Int focused nuking.

Bokor is terrible with Lycanthrophy, IMO.
Lycanthrophy is full of low/no CD skills. You're better off relying on persistent effects from other builds (buffs, statues, ground AoE) rather than a class that already revolves around spamming a no CD skill. Wasting 3 ranks for mack and samediveve alone seems like a huge waste compared to what you'd get from going Diev3.
If you still don't want Diev3 for whatever reason, I'd still pick Priest3 over Bokor3.

You don't have to max either, though it's generally advised to max Effigy.
You just can't leave them at level 1 and expect them to be useful. Hexing's duration is specially important in PvP, but shouldn't be neglected in PvE either. You have to remember it's actually pretty easy to resist hexing from stats alone.

It's up to you. I use both, and though I rarely get any large zombies due to how rare large mobs are the extra CD doesn't bother me too much with 15 Zombify and most of my zombies tend to come from Hexing anyway.
If you didn't know, if a zombie gets the killing blow on a mob affected with curse, it spawns another zombie.

How the fuck do people stay commited to a single character or continue grinding? I've made literally dozens of characters and none of them have gotten past 160. It's probably because I'm super bored from grinding quests or missions or the boring dungeons (110 and 145).

I don't think Ogouveve was active when I took that screenshot. Usually with Ogouveve active they deal somewhere around 500-700 per hit for me and there's up to 19 of them hitting fairly rapidly.
That said, the damage is really not that amazing and you shouldn't be expecting to top DPS charts or whatever but I don't think it's anywhere near as bad as people make it out to be.
