Once you pass 200 you gain the ability to Guilt medals. Dots themselves increase the damage multiplier of the medal's special up to five dots. Once you add 6 dots onto a medal(That means a single medal + 6 of the same type of medal at the same level, has to be 6* for more than 3 dots) you gain an extra multiplier % of damage on the thing. Here's a picture on how to cost effectively do it.

You'll be doing it for a medal like Stitch guaranteed.
