It isn't. Looks like a metal plate to reinforce the knee. As you grow older the bone grows around the plate making it harder to remove. While that shows something to the extreme it's pretty common for it to be hard to remove.
Also it doesn't damage any part of you.

I initially started training farming back in like 2010 as a way to distract myself from training the skills that you couldn't afk back then (agility/thieving/mining). 80 minutes was about as long as I could stomach doing those at a time, so I'd do an herb run, train those skills until time was up, then do another herb run. Eventually fell in love with the skill and it was my second 99. Stopped trees after 99, but kept doing herb runs every so often for easy money.
