I'm guessing he foresaw this game wouldn't be finished, so built the concept around a phantom pain from an incomplete game. Even the characters are symbolic.
Huey = Kojima
Skullface and XOF = Konami
MGS fan base = Diamond Dogs

He wasn't trying to be subtle when Huey wore his glasses towards the end. Kojima was forced to finish a project, much like Huey was forced to finish Sahelanthropus. In the end, he wasn't needed as it could be completed without him, so Skullface (Konami) gets rid of him. Huey is put on trial by Diamond Dgos for the mess he made, much like Kojima was by the fan base for this mess of a game. In the end he gets exiled off, and we are all left with a "phantom pain" of an unfinished story.

Did he know in 2013? Because trailers from that far back feature every cutscene in the game. Huey with the glasses and all. So the most difficult parts were done years before release.

Here is what happened. Hideo purposely wasted everyones time and money. He had some ideas for cutscenes, and he made them with his favorite actors. He lived his dream. Then took a three year vacation. Instead of directing the gameplay, he was out on location making tweets and taking hamburger pictures. Konami forced him to do his god damned job, and direct the game.

If bratty hideo had his way this would have been a canceled game since he was drawing out the development for so long.

Here's what really happened:

We were promised a grimdark story, and 'Ground Zeroes' was to prepare us for it.
But then the ''GZ' is a paid demo' fiasco caused Konami to fear 'TPP' would not sell as well, and that a grimdark story would make it sell even less or just cause them trouble, so they told Kojima to create a very safe story to make a safe dollar.

So Kojima heavily edited the story to meet their demands and that's how 'TPP' ended up as it is.

Really, it happened before with the video game 'Haze', the publisher made them change crazy amounts of shit, like change it from third-person shooter to first-person shooter, change the story from Middle East to Africa, from moral ambiguity about freedom (rebels) and peace (military occupation) to the military occupation is evil.
