or alternatively, if you are a Pyromancy character you can just chuck Great Chaos Fire Orbs at him until he dies.

I got him on my second try, it was laughable. You get him to second phase through careful punishes, and then you gotta haul off with as many Chaos Fire Orbs as you can, you'll do stupid amounts of damage to him with the three Pyromancy Rings and 30/30 INT/FTH. The orbs should straight up kill his ghost before it even has a chance to move because of the lava, and he will die where he started charging up.
