On top of that you should drop Rage, just go for a single Crit item, either Malice or DB. Titans is always a good pick, Winged blade is still a great defensive pick if the enemy teams have slows and Masamune could be an alternative to SCS if you value the movespeed while giving you good power especially since you have relatively low max health. Brawler is pretty good against heal oriented team and new Hydra can tie up a build giving you more CDR and its passive having pretty good synergy with yours for bigger bursts.

You're a shit thanatos if you allow yourself to be focused. Thanatos comes in mid-fight. The only time he starts fights is if it's a gank. When it comes to actual team fights, he doesn't start them like Thor can. Also, if you get caught you just ult out and reset.
