Mostly making it so that mutations have more of a presence to them and a point. Maybe also tying weapon strength to enemy strength so legendary glowing deathclaw fucksup matriarch kings no longer just give you rolling pins. With the way things are going, assuming the Prydwen being killed isn't canon, I can see the East Coast becoming a sort of pseudo-feudal kingdom under Maxson as opposed to the democracy of the NCR on the West Coast.

I'd love for a DLC to have you visit part of the Capital Wasteland, I'm interested to see what the BoS have done in the past decade, it must be reasonably secure given their military commitment to the Commonwealth but I wonder if it's develop it's infrastructure in the decade of relative peace and security.

The BoS is advanced but all their tech is military, they don't have the core of civic technologies and experts that the NCR had with Vault City.

It's funny but since Maxson's politics are essentially identical to Colonel Autumn's, really it would have been better for the East Coast if the Lone Wanderer never left the vault, the Enclave would had won and Colonel Autumn would have destroyed President Eden and the FEV virus.
