I can assure you I'm not in NG+. I guess I was a bit gimped for Ludwig as I was using a Hunter's Axe (+8 I guess) with like 30 Vit and 30 Str, with 25 in BT which was wasted at that point. But by Orphan (level 100) I had my Bloodletter and 40 Vit so I can't really explain why I wouldn't have had that overlevelled stomping experience if what you say is true.

Again, it's possible that I'm just truly awful, but I don't think that's the case.

No. Old Hunter DLC is pretty much end game. You can do it earlier but if you do it at the right time (just before finishing the game) you'll easily be at mid 80's when starting.

I'm not very far, I just beat the Bloodthirsty Beast and got caught in the Gaol, any real tips? I like the Tonitrus I found but its durability a shit. I also have Hunter Axe, Saw Spear, and Kirkhammer and I like them, but not sure. I've mostly been putting my stats into VIT, END, and STR with enough Skill to use Kirkhammer. I feel like my lefthand weapons all suck but if I am going for a Strength build that seems to be about the usual issue.

What tips can you suggest?

For more damage, what you need are more gems and upgrade mats to hit +6. The Gaol is a side area, so you're probably pretty done with already if you've done some exploring.

There are some good gems as loot as you progress through the story, or you could hit up some chalice dungeons if you want at this point.

Builds are really simple in this game, just pick a weapon, an ending level, put points in the applicable stats, and put everything else in vit and maybe some end.
