In town, infected heroes only spread the cancer around if they are using a same building as uninfected heroes. In 2-3 weeks they will spend one blood resource from your pool if they must, or they die of starvation. In combat they might cause some additional stress to your pals or even attack them if they are thirsting for blood. But when given some blood, they get a pretty good speed and damage boost for like half a dungeon. I didn't buy the dlc immediately, but I'm glad I did after all.

I just killed the Baron and now I went through 10 runs on other dungeons without even encountering a gatekeeper
I hardly encountered Courtyard monsters at all

Also, her skills are bad. She's stronger than Leper so the creator "evened her out" by making her own skills give her bleed on TOP of moving her position. Every one of them. So you either need a highly flexible/mobile group or you're fucked.

Dont play arround Death's door, while it sounds pretty op is better not to risk it.

Play safe with +bleed or + debuff trinkets and Book of rage.

Dont bother with the backrow attack, use Punish-Exsanguinate-Redeem-Suffer and replace Suffer for Endure when you need to stress heal.

Rapturous; he cant get ANY virtue, he got his own affliction which boost him alot, still the trade off is heavy stress and worse: he will whip your own party members and bleed them on top. One of his main weak points is how quickly he can reach well over 100% stress if you dont watch out.

He is all about sucking damage away from his team and healbombing/heavy bleeding enemies, he is pretty strong as long you can bleed.

He is pretty fun and the best part of the DLC

It could still work, probably. City is quarantined because of the eldritch shit, so you have to make due with the couple survivors you find and anyone stupid enough to sneak in to help you. Different districts are more urban like parks and the waterfront or something. Ammo is scarce so characters that use melee can still do so justifiably. It would take some explaining, but it's not impossible.
