1: Don't assume you'll make WoW grade success. Expect a few hundred thousand concurrent subs to be a success.
2: System requirements should be kept low. MMORPG players do not mind a game that doesn't push their system to it's limits, and by keeping things simple you expand your potential player base.
3: See those Russians? Brazilians? They play on their own servers if they can't speak the regional language of the server.
4: The game doesn't play itself for you. No fucking queues except for certain very specific events that call for it- like holiday events and tournaments. Players should never be in a position where the optimal choice for play is to push a button and wait their turn in line.
5: No cross server anything. No paid transfers. Transfers are only offered when a server is being shut down or to try and populate a dying server.
6: Player interaction tends to be the backbone of any good MMORPG. Solo content needs to be handled carefully.
7: Economy should tend to be favored towards player run options. This means crafting needs to not be dick.

There's no such thing as a MASSIVELY multiplayer online RPG anymore. Old games got big because that's all there was; it was innovative and everyone played the same few games. Now the gaming population has far more choice and it's impossible to attract an audience both as large and as dedicated as you would need to sustain a living online world. These days they're normal multiplayer RPGs where you can see other players in the towns and overworld.

Make players dependant on each other and encourage them to cooperate and socialise. Make players value their possessions , upgrades should be a big moment and the result of hard labour. Focus the downtime and uptime equally. Relaxing should be as enjoyable as engaging with the games mechanics.
