Really, there are so many ways to have made MGSV's ending better.
>Big Boss has gone crazy, there was never a medic, or a Skullface. The only thing that still makes sense to Big Boss is war. After making amends with Zero, Big Boss meddles with the Patriots AI in order to cause wars, so he can have Outer Heaven get involved in them. This is what eventually makes the Patriots go nuts in MGS4.
>Or, Venom Snake wants revenge on Big Boss, and started Outer Heaven as a way to ruin his reputation. Big Boss came to realize The Boss's will, and ran The Patriots and Foxhound between MGSV and MG as a peace keeping organization. The Patriots AI betrays Big Boss by propping up Venom Snake and Outer Heaven. Big Boss sends in Grey Fox and later Solid Snake in order to put an end to Venom, and then does all the shit in Metal Gear 2 in order to stop the Patriots AI like Solidus in MGS2 and Ocelot in every MGS game, but Kaz tricks Solid Snake into being the instrument in his plan to get revenge for being lied to in MGSV.

But instead we got:
