Get MGE XE, Morrowind Code Patch and official Bethesda plug ins. These are just bug fixes and improvements on vanilla.
So play vanilla first, get used to the game, then just go wild and install a shitload of mods as always. Trainwiz mods are great btw
I recommend the Morrowind Watercolored texture pack for a slightly better looking vanilla without much effort.

Whatever but for fucks sake keep an eye on your stamina meter as it affects anything. Running 10 miles and trying to shoot someone with a bow while wheezing and panting is just not gonna go that well

Oh, also better eyes and a fun mod that simulates eye animations for NPCs. Basically their eyes move in the opposite direction of your cursor or something, so they look at you when you're pointing at them, and as you move away so do their glances. As the author pointed out that makes for a funny situation where if you stare at a woman's tits it'll look like she's rolling her eyes up in disappointment

It's really useful. Though even without it the game is still less broken than the later Elder Scrolls games.

This and some UI fixes and HD text. Engilsh signposts is also great. I personally enjoy the mod that replaces the vanilla ghostfence with the concept version since it looks better.

Pretty much this. I actually re-installed it a week ago because it's been like 5 years since I played it
I decided to play it vanilla because that's the version that I fell in love with it (and many many many other people)
Best decision I ever had
Just used Morrowind Code Patch to fix bugs and stuff like that
and MGE XE to add some eyecandy, even in this regard i only used anti aliasing and anisotropic filter
Also made the render distance a (generous) double from the vanilla morrowind, the fog actually masks stuff intellegently and contributes to the feel of discovery
It's kinda useless when you can see the whole region from a hill, it looks like a minature set (the original was way too short, so something like 2.7 it's perfect)
Also be careful because I saw too many people add SSAO, HDR and whatever and end up playing at 15 fps
You could use OpenMW but I still think that as of today Vanilla Morrowind+code patch+mge xe is perfect

Also a running/walking speed mod, if youre coming from skyrim or oblivion you'll feel like a snail
Leave actual modding for a later playthrough, as this games have actual factions that lock you out from guilds, youre guaranteed to play again if you want to play all quests
This is unlike oblivion and skyrim where you can be master at every guild no matter how conflicting their goals or values are

Yep, I love every mod that adds concept art aesthetics and clutter, Main Quest Enhancers is great for that too.
