Generally act 1 will take the structure of all the original acts of the zone and mix them around while also adding some new stuff, and act 2 will be completely new.

In the Sonic CD levels, even Act 1 is made from scratch, because Sonic CD has crazy level design.

There's 8 old zones and 4 new ones for a Total of 12, less than Sonic 3 and Knuckles but more than any standalone classic sonic game. Four zones might not seem like much new stuff but with every second act of the old zones being completely new there's a lot of new stuff, and even the old stuff isn't one for one remake.
