It's been pretty well documented what went wrong.
They spent a hell of a lot of dev time trying to get procedual planets to work only to scrap it all last minute and cobble together what we got in a year.

It's still hilarious how there are no white people in this game and most of the power is in the hands of women.

Maybe because I waited several months after release for animation fixes, and better character customization, but it wasn't nearly as bad as everyone made it out to be.
