Imho Karma system should work like 10 points system oWoD Roads (see http://whitewolf.wikia.com/wiki/Road_of_Humanity) .

Few quest should allow you to actually "Gain Karma" points and it should be used like another skill in dialogues, to get special side quests and as an initial reaction modifier (bad NPCs for example would ignore you or shoot you instead of giving you a quest if your karma is too high)
