I think there's a good idea there but I dunno if an RPG would be the best fit. You could have an action game where combat is very simple, but made harder and more intense from having next to no visual cues to work off. Everything that isn't making noise or somehow obvious to the character is completely black, and the environments are built to have as many noisy things in them as possible. Things appear and disappear as they make noise/go quiet, including enemies. Might b kewl.
