You can do all the sidequests I think, but you can't talk to certain NPCs and are often forced into a stealth playthrough.

You can walk on the surface like any other clan, just don't walk straight into non-quest NPCs and you will be allright.
Most quest NPCs have unique dialogue for nossie which can be very amusing.

I rarely bother with the sewers. Just stealth along the side of the streets avoiding people. And yeah, side quest givers still talk to you. They just comment on your costume, or leprosy, or whatever they assume is wrong with you.

I know that the guy in hollywood, the one that's writing the movie script about vampires shits himself the moment you initiate conversation, trying to talk to him just results in him groaning in terror. Same with the fat guy in Santa Monica that handles the bounty hunter quest, not sure if any of these prevent the quests from being completed though, it's been a while since I've last played.
