SH3's otherworld is Claudia's otherworld, reason it appears in another town like in the mall and hilltop center is because she has the ability to spawn it, how and why there is no explanation, but Valtiel turning the valve is what causes the switch to the otherworld.

in SH2, the manifestations are unique to each individual, as they themselves are the sources of the projections. Occasionally these "mind bubbles" of two people collided, resulting twisted mutations in the eyes of the MC.

In SH1, 3 and 4, the MC and side characters are all witnessing one, major "alpha projection", originating from a strong source. They personally have very little to literally no effect in what they're witnessing.

The OG "cult" was just a pack of extremist Christians who'd adapted some pagan rituals, because that shit actually did something. Some extra lore was twisted out of existing mythologies to make them click better with the bible-folks. They still believed in THE God, and Dahlia merely wanted to ignite the promised apocalypse by birthing a living, breathing God.

SH3 cult on the other hand had gone through a mutation sometime after #1, now focusing totally on worshipping Alessa as the "god".
