The irony is that Super Mutants continue to fight and kill each other centuries into the future, even though the Master wanted to create a perfect race that would usher in a new age of peace and prosperity. It's not made clear, and the problem got especially worse with Fallout 3 bringing up androgyny in its description of Fawkes. There's at least one mutant in Fallout 1 who has a girlfriend, but whether they have a sexual relationship or not is completely debatable. As tempting as it is to think that "everything" gets bigger just because most of the body does, their inability to reproduce makes it very questionable.

Fallout 1 mutants are mostly normal since they used mostly high quality subjects with no mutations. The dumb dumbs are from Fallout 2 where the enclave turned a bunch of miners into super mutants and they werent exactly pure of mutations.

Women were turned into mutants regularly in Fallout 1. Saying that they're all "gender neutral" because their sexual organs fell off was Fallout 3's attempt at hand-waving the lack of noticeably female Super Mutants (though even that's fuzzy since the FEV of Fallout 3 is different than Fallout 1's).
