It's a hard balance. If you put a game on the hardest difficulty, it usually means you have to really really use the games mechanics. Firefights become harder, so you have to use more of the special abilities. On easy, you can just run and gun without worry about dying at all. But if it's on hard, you have to use every ability you can.

THAT SAID

A lot of games end up being worse if you put on the hardest difficulty. You end up having to use extremely cheesy tactics in order to win. You end up using the WRONG mechanics. Trying to get enemies pathfinding to fuck up, taking advantage of choke points or the fucking worst...
THE FUCKING WORST is backpedaling when fighting. You have to do that shit all the time because enemies turn into bulletsponges. So many fucking FPS games turn into "RUN BACKWARDS ALL THE TIME."

It should be challenging enough for a first and maybe second playthrough, Hard should be a real challenge and not just more enemies with more bulletsponginess while you do less damage or have % less chance to hit but then designing around that can be a real challenge, preferably it would actually change the goals, game mechanics, world locations, enemy behavior etc.

Thief's Expert difficulty is literally perfection but that's not applicable to many games, clever design is though.

Sky Odyssey on the PS2, having all the flight assists on makes it impossible to do a lot of the missions in the main adventure mode. So many missions depend upon you being able to navigate tight corridors, manage throttle and gliding, even using wind currents to ascend cliff faces, and that isn't even counting acrobatics. you can play witcher2 on easy and learn to roll and use shield magic and then switch to dark and it's the same core gameplay, no real difficulty increase, or you can play witcher3 and just learn to use the quicktime dodge to never take damage and it's like playing on easy

you can 3d arpg shooter and just hide behind covers 99% of the time or stock up on healing items and heavier armor and it's like there's no change

so why choose harder difficulties if you can just play on normal but choose to play the game like a mad berserker who just charges at enemies or only using pistol/weak weapons for example, creating your own, fun, dynamic difficulty

Normal for first playthrough, if I like it enough for a second round then I pick Hard. I reserve Easy for the final playthrough with all the bonus costumes and unlockable cheats for some dumb fun.

I usually pick normal, unless for some reason I think a game would be more fun on hard.
Thanks to most devs having no idea how to make a hard mode fun, I switch back to normal soon enough.

I loved DOOM on hard, but Prey on Hard was one of the most un-fun experiences I've had in a long-ass time. Managed to play up until the Arboreum before dropping difficulty.

From mode is normal mode but you die in one hit, its the mode the game touts as the biggest challenge. Hard mode is a lot worse honestly

I only ever play on the hardest difficulty because if i cant play without a challenge, then whats the point of playing? Unless its artificial difficulty, then the game is basically preventing you from advancing. For example, splinter cell conviction. Enemies can see through walls and know exactly where you are at all times. Thats artificial shit. Lets compare spec ops: the line. I play FUBAR and its no different than the other difficulties, because each difficulty only increases the demand for more accurate hand eye coordination and dexterity, and reflexes. Thats real difficulty.

All in all games come down to "do you have the ability to play?" "do you want a challenge". "are you going to test yourself to beat the game?" The game is only ever as hard as the player is stupid.
