They're gas clouds, usually pretty massive. The mass is so great, the gases undergo fusion and create stars.
They were formed after the big bang, the plasma cooled and formed massive hydrogen gas clouds. Like 98% of the universes total mass is hydrogen.

Nebula are giant clouds of gas in space. Smaller ones are usually leftovers from supernova. Larger ones (like the pillars of creation) are leftovers from the galaxy forming. Chances are low.
Rogue planets get nearly no light, and therefore almost no outside energy. Theoretically if it was very chemically/volcanically active it could support life, but it would be very different from what we know.
There are also some very theoretical types of "life" (silicon based, crystal formations etc) that might function okay on those planets, but since we're not even sure those kinds of weird ideas would work, the best answer for now is "no"

Neutron stars are relatively rare, so the chances of that happening are kind of rare. Then again scientists are looking for graviton waves from black holes colliding, which should be even rarer.
I've never found a source as to what happens when two massive objects come into contact with one another. I'm fairly sure stars colliding doesn't just add mass though, its usually pretty disastrous for both.

Learn to use the LOD settings. If you're taking anything that focuses on a planet they are essential, you can actually push them above the slider in game with the console (makes the game chug though)
After that its just a matter of playing with the other settings and positioning until you like what you see.
Handy tip: you can speed up or slow down time to get that sunrise or to line up the planets how you want.
