Yes, it's all but confirmed to be happening. Shibusawa has said to KT investors during a shareholder's meeting that there's "A future for Nioh as a franchise", and Yasuda has said that the team are brainstorming ideas for a sequel right now and will likely go into the planning stages after the 3rd DLC's finished. Sony will probably offer some extra financial motivation for a sequel too, since the original sold very well, and they're after some exclusives to keep nerds and weebs onside.

It's just induction. It made a lot of money relatively speaking for KT games.

If not a direct sequel, it's for certain getting a sort of spiritual successor. Did incredibly well for being a very niche new IP. Have read that Koei and Team Ninja are both very pleased with how it turned out, they also seem to like William as a character.
Easily my goty too. Shame it can't be discussed here much, but that's likely for the better

Literally never. They were only going to entertain Ninja Gaiden 4 if Nioh did poorly. Since Nioh did well they want to continue it as a new franchise. I guess we all shoulda done our part to show them we want more Ninja Gaiden by buying Yaiba.
