King's Quest series. Those games have actual bullshit deaths but at least allows you to retry and reload a previous save file. The real nightmare is when the game gets stuck on an unwinnable situation because it never tells you about it.

Fall damage in Witcher 3 was inconsistent and kind of silly. You can slide down the entirety of a mountain and survive but jump off a two foot tall object and die after getting stuck in a falling animation
