Managed to do that in Nier Automata. Once I finally managed to get through the final boss I couldn't get through the opening sequence for NG+. I'm actually pretty sure it was physically impossible to beat at the level I had and I didn't have any items. Technically could have lowered the game level, but it's a great game so I just made a new file and didn't skip everything I could that playthrough.
