I hate this paradigm.

I want a game that will kill you for not choosing the correct class, "healer".

It would play like Trauma-Center meets Dark Souls, meets "Dangerous Game" or the monster-hunting sidequests in the Witcher, Basically you'd wander into a monster-infested area, try and provide medical assistance to survivors, and shoot any other monsters with silver bullets (which are expensive and you're encouraged to attempt to retrieve for reuse, also monster-carcases/body-parts can be somewhat valuable, so you might be able to afford missing/losing bullets depending on the payout).

I guess another way, Picture the witcher with less sex or intrigue but more business management and occassional STALKER-ish moments of helplessness.
