Metroidvania originally described a Castlevania game that plays like Metroid. It was expanded (or rather, misused) to describe ANY game that plays like Metroid, but even if we accept this defintion, La Mulana is still not a metroidvania because it has too much of a puzzle emphasis with elements of action, whereas Metroid is an action emphasis with elements of puzzle.
"You get items to progress in the game that aren't simple keys" doesn't make it like Metroid, it makes it an adventure game. Metroid's "keys" differ from traditional adventure games because Metroid is a platformer and almost all of Metroid's "keys" are new mobility options, where La Mulana is sorely lacking
